16 de Junio 2020.- Con la articulación del Programa Subregional para América del Sur de la OPS, se llevó a cabo hoy una reunión virtual entre expertos de del Proyecto ECHO (Extension for Community Healthcare Outcomes) de Uruguay, y el Ministerio de Salud del Perú, quienes intercambiaron experiencias en tele capacitación y tutoría a distancia de profesionales de la salud en zonas precarias y de difícil acceso. El Proyecto ECHO de Uruguay, está en la Facultad de Medicina de la Universidad de la República y desarrolla la tele capacitación para profesionales rurales y de comunidades fronterizas, con el apoyo de especialistas de centros hospitalarios.

La reunión de hoy, a través del auspicio del Programa Subregional para América del Sur de la OPS, buscó brindar un espacio para la transferencia de conocimiento entre Uruguay y Perú, para que el Ministerio de Salud del Perú pudiera establecer un programa similar de tutoría para profesionales de la salud trabajando en el primer nivel de atención.

Esta reunión concluyó con la decisión de formalizar un acuerdo interinstitucional entre ambos países para apoyar la implementación de un programa de tutoría a profesionales peruanos que están siendo contratados para atender a la población por COVID 19 tanto en el primer nivel como en los hospitales.

La Organización Panamericana de la Salud, a través del Programa Subregional para América de Sur, coordinará este acuerdo y brindará la asistencia técnica necesaria durante todo el proceso, junto con las oficinas de país de OPS/OMS en Uruguay y Perú.

La reunión contó con la participación de representantes del Ministerio de Salud del Perú (Dirección de Personal de Salud, Dirección General de Tele salud, y la Dirección General de Prevención de Riesgos y Desastres), la Universidad de la República del Uruguay, el Programa Subregional para América del Sur de la OPS, las oficinas de OPS de Perú y Uruguay y la Universidad Nacional Mayor de San Marcos. En el encuentro, se llevó a cabo un intercambio de experiencias entre el proyecto ECHO Uruguay representado por el Dr. Henry Cohen y el Ministerio de Salud del Perú.

El webinar es una invitación a reflexionar sobre cómo las distintas experticias profesionales se combinan para ofrecer el cuidado interprofesional en salud y producir una sinergia en bien del paciente

El Dr. Ricardo A. Ayla es miembro del personal de investigación en Ghent University (Bélgica), profesor de sociología de la salud en el International Bachelor in Social Sciences de la Universidad Libre de Bruselas.  Autor de una bibliografía prolíficua en el campo de las profesiones, combina su propia experiencia profesional con su área de especialización académica. Tras una carrera en gestión de salud en Chile, cursó sociología en Université de Strasbourg (Francia) y presenta artículos publicados en revistas como Journal Interprofessional Care, Social Theory & Health, Medical Humanities, Nursing Inquiry y Archives of Medical Research.

Resumen: El informe sobre el estado de la enfermería en el mundo 2020 proporciona la evidencia y las opciones de política más recientes y actualizadas para la fuerza laboral mundial de enfermería. También presenta un caso convincente para una inversión considerable, aunque factible, en educación de enfermería, empleos y liderazgo.

 

Enfermeiras e enfermeiros representam mais da metade de todos os profissionais de saúde do mundo, fornecendo serviços vitais em todo o sistema de saúde. Historicamente, esses profissionais estão na vanguarda do combate a epidemias e pandemias que ameaçam a saúde. Em todo o mundo, estão demonstrando sua compaixão, bravura e coragem ao responder à pandemia de COVID-19: nunca antes seu valor foi demonstrado com tanta clareza.

““Enfermeiras e enfermeiros são a espinha dorsal de qualquer sistema de saúde. Hoje, muitos desses profissionais estão na linha de frente da batalha contra a COVID-19”, afirmou Tedros Adhanom Ghebreyesus, diretor-geral da OMS. “Este relatório é um lembrete direto do papel único que desempenham e um alerta para garantir que obtenham o apoio necessário para manter o mundo saudável”.

O relatório, lançado pela Organização Mundial de Saúde (OMS) em parceria com o Conselho Internacional de Enfermeiras (ICN, sigla em inglês)) e Nursing Now, revela que hoje existem pouco menos de 28 milhões de enfermeiros em todo o mundo. Entre 2013 e 2018, os números de pessoal de enfermagem aumentaram 4,7 milhões. Mas isso ainda deixa um déficit global de 5,9 milhões – com as maiores lacunas encontradas em países da África, Sudeste Asiático e da região do Mediterrâneo Oriental (da OMS), além de algumas partes da América Latina.

De acordo com a OMS, é revelador que mais de 80% das enfermeiras e enfermeiros do mundo trabalhem em países que abrigam metade da população mundial. Um em cada oito profissionais de enfermagem trabalha em um país diferente daquele em que nasceu ou foi capacitado. O envelhecimento também ameaça a força de trabalho de enfermagem: espera-se que um em cada seis enfermeiros e enfermeiras do mundo se aposente nos próximos 10 anos.

Para evitar a escassez global de profissionais, o relatório estima que os países que sofrem de escassez precisam aumentar em média 8% o número total de graduados em enfermagem por ano, juntamente com a capacidade aprimorada de empregar e retê-los no sistema de saúde. Isso custaria aproximadamente US$ 10 per capita por ano.

““Políticos entendem o custo de capacitar e manter uma força de trabalho profissional de enfermagem, mas somente agora muitos deles reconhecem seu verdadeiro valor”, disse a presidente da ICN, Annette Kennedy. “Cada centavo investido em enfermagem eleva o bem-estar de pessoas e famílias de maneiras tangíveis e claras para todos verem. Este relatório destaca a contribuição da enfermagem e confirma que o investimento na profissão é um benefício para a sociedade, não um custo. O mundo precisa de mais milhões de enfermeiras e enfermeiros, e estamos pedindo aos governos que façam a coisa certa, invistam nessa maravilhosa profissão e assistam suas populações se beneficiarem do incrível trabalho que somente estes profissionais podem fazer.”

Cerca de 90% de todos profissionais de enfermagem são mulheres, mas poucas delas estão em cargos de liderança – a maior parte deles é ocupada por homens. Contudo, quando países que permitem às enfermeiras assumir papeis de liderança, como chefe de enfermagem (ou equivalente), e programas de liderança em enfermagem, como condições melhores para os profissionais da área.

““Este relatório apresenta dados e evidências muito necessários para fortalecer a liderança da enfermagem, aprimorar sua prática e capacitar essa força de trabalho para o futuro”, afirmou Lord Nigel Crisp, copresidente da iniciativa Nursing Now. “Nós acreditamos que opções políticas refletem ações que acreditamos que todos os países podem realizar nos próximos dez anos para garantir que haja enfermeiros e enfermeiras suficientes em todos os países e que esses profissionais usem todos os seus conhecimentos, capacidades e escopo profissional para melhorar a atenção primária à saúde e responder a emergências de saúde, como a causada pela COVID-19. Isso deve começar com um diálogo amplo e intersetorial, que posiciona as evidências de enfermagem no contexto do sistema de saúde de um país, força de trabalho em saúde e prioridades de saúde.”

Para equipar o mundo com a força de trabalho de enfermagem do qual precisa, a OMS e seus parceiros recomendam que todos os países:

• Aumentem o financiamento para capacitar e empregar mais enfermeiras e enfermeiros
• Fortaleçam a capacidade de coletar, analisar e agir sobre dados referentes à força de trabalho em saúde
• Monitorem a mobilidade e a migração de enfermeiras e enfermeiros e gerenciá-las de forma responsável e ética
• Eduquem e capacitem enfermeiras e enfermeiros em habilidades científicas, tecnológicas e sociológicas necessárias para impulsionar o progresso na atenção primária à saúde
• Estabeleçam posições de liderança, incluindo enfermeiras em cargos de chefia, e apoiem o desenvolvimento da liderança entre jovens enfermeiras.
• Garantam que enfermeiras e enfermeiros das equipes de atenção primária à saúde trabalhem em todo o seu potencial, por exemplo, na prevenção e manejo de doenças crônicas não transmissíveis
• Melhorem as condições de trabalho: níveis seguros de pessoal, salários justos, direitos à saúde e segurança ocupacional
• Implementem políticas sensíveis às questões de gênero para a força de trabalho de enfermagem
• Modernizem a regulamentação profissional da enfermagem, harmonizando padrões de educação e prática e usando sistemas que possam reconhecer e processar as credenciais desses profissionais em todo o mundo
• Fortaleçam o papel de enfermeiras e enfermeiros nas equipes de assistência, reunindo diferentes setores (saúde, educação, imigração, finanças e trabalho) junto às partes interessadas da enfermagem para o diálogo sobre políticas e o planejamento da força de trabalho

A mensagem do relatório é clara: os governos precisam investir em uma aceleração massiva do ensino de enfermagem, na criação de empregos e na liderança. Sem enfermeiras, enfermeiros, obstetrizes e outros profissionais de saúde, os países não podem vencer a batalha contra epidemias ou alcançar a saúde universal e os Objetivos de Desenvolvimento Sustentável (ODS).

Entre las medidas necesarias para la prevención y el control de la infección por COVID-19 promovida por la OMS, el curso enfatiza las acciones para reducir el contagio entre las personas y las prácticas para identificar, aislar e informar casos sospechosos y confirmados. Al final del curso, los profesionales de la salud deberían ser capaces de:

• Definir CPI y su rol en términos de preparación, disposición y respuesta;
• Describir la situación epidemiológica actual del COVID-19, con definiciones de los casos, signos y síntomas incluidos;
• Definir el control de la fuente, los controles administrativos y los controles técnicos y de ambiente;
• Conocer las medidas de prevención y control de la infección, recomendadas por la Organización Mundial de la Salud para los centros de salud, incluidos los casos de sospecha o confirmación de COVID-19;
• Establecer y poner en práctica medidas complementarias de prevención y control de la infección para colaborar en la preparación general del centro de salud.

El Plan de Acción está organizado en objetivos, indicadores y atributos para abordar las tres lineas principales de la Estrategia Regional de RHS: 1) fortalecer y consolidar la gobernanza y rectoría de los recursos humanos para la salud ; 2) desarrollar condiciones y capacidades de los recursos humanos para la salud para ampliar el acceso y la cobertura de salud con equidad y calidad ; y 3) concertar con el sector educativo para dar respuesta a las necesidades de los sistemas de salud en transformación hacia el acceso universal a la salud y la cobertura universal de salud.
